Agreement of Release and Waiver of Liability

I voluntarily agree to assume all risks and responsibility for any injury or accident that might occur to me or my child/children during any of the Zumba Kids, Little Champions or Tiny Tumblers classes, rehearsals, performances, or activities. I also exempt, release, and indemnify Little Champions, Zumba Kids, Tiny Tumblers Sara Leonardo, or Employees of Kids Rock Fitness, Kids Rock Fitness and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, or injury to my child/children, or property which may arise out of or in connection with participation in any classes or activities conducted by Sara Leonardo or Employees of Kids Rock Fitness. I further voluntarily agree to waive my rights and those of my heirs and assigns to hold Sara Leonardo or Employees of Kids Rock Fitness liable for such damage, loss, or injury. I represent and warrant that my child/children have no known medical condition that would prevent his/her/their full participation in the Zumba Kids Fitness, Little Champions or Tiny Tumblers class. I am signing this waiver for my child/children. I certify that I am the parent or legal guardian and have the right to waive these rights.

There is no refund for missed classes. 

Sara Leonardo has my permission to use my child's/children's photograph to promote Zumba Kids/Jr. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty fee, or other compensations shall become payable to me by reason of such use. PLEASE NOTE: I have no plans to post pictures. Waiver is for class picture. If a photo was to ever be used, I would get parent's consent before.
